What is Facebook ad creative fatigue?

Short answer

Creative fatigue is when an audience has seen the same ad so often people may engage less, which can raise cost per result. For ad sets with one creative, Meta flags it in the Delivery column: Creative limited when costs exceed your past ads, Creative fatigue at twice or more. Fix it with materially different creative or a wider audience.

Frequency is the average number of times each person saw your ad. Meta's guidance is that if performance drops as frequency rises, your audience may be tiring of the ad, and it may be time to change the creative or the targeting.
